Colostethus leucophaeus Duellman, 2004, Sci. Pap. Nat. Hist. Mus. Univ. Kansas, 35: 26. Holotype: KU 211879, by original designation. Type locality: "Molinapampa (06° 11′ S, 77° 38′ W, 2400 m), Provincia de Chachapoyas, Departamento de Amazonas, Peru".

Known only from the type locality at an elevation of 2400 m in the northern part of the Cordillera Central in Departamento de Amazonas, Peru.
