Atelopus eusebiodiazi Venegas, Catenazzi, Siu-Ting, and Carrillo, 2008, Salamandra, 44: 164. Holotype: MUSM 7138, by original designation. Type locality: "Huamba, 22 km ESE (airline) of Ayabaca (4° 43′ 7.2″ S, 79° 31′ 50.88″ W) at an elevation 2,950 m, near Cerro Mayordomo, Provincia de Ayabaca, Departamento de Piura, Peru".

Distribution
Known only from the type locality in the northern portion of the Cordillera de Huancabamba, Departamento de Piura, northwestern Peru, 2950 m elevation.
