Gastrotheca phalarosa Duellman and Venegas, 2005, Herpetologica, 61: 299. Holotype: MUSM 19487, by original designation. Type locality: "Esperanza (06° 49′ 10.1 S, 77° 43′ 21.7 W, 3435 m), Juanjui, Provincia de Huallaga, Departamento de San Martín, Peru".

Distribution
Known only from the type locality (Esperanza) in the vicinity of Laguna de Los Condores in the northern part of the Cordillera central in northern Peru, 3435 m elevation.
